This article is about the non-extraction orthodontic treatment of an adult who had a severe crowding by distalization of upper and lower posterior teeth with lingual appliance and skeletal anchorages. A 37-year-old male patient demanded orthodontic treatment with invisible appliance and wanted to maintain his facial profile. The lingual appliance (Incognito, 3M Unitek, Bad Essen, Germany) was chosen. Distal traction of upper and lower posteriors was performed with skeletal anchorages. As a result of 32 months orthodontic treatment, proper amount of distalization was achieved. In an overall superimposition, total amount of distalization for lower posterior was 3.0 mm and 2.0 mm for upper posterior teeth. The ideal occlusion was achieved maintaining patient’s profile. The mandibular position was maintained as planned. Distalization with lingual appliance and skeletal anchorages was successfully performed with maintaining patient’s profile.
