This case report presents an effective surgically assisted orthodontic treatment option for the correction of convex profile through anterior segmental osteotomy (ASO) and premolar extraction in a senior patient with an anterior dental implant. A 56-year-old man with a previously installed dental implant in upper anterior teeth was treated with ASO using the IncognitoTM system. Extraction of four premolars with ASO was performed in the maxilla and mandible to improve facial convexity. Treatment results were achieved within 12 months with a combination of ASO-assisted customized lingual orthodontic treatment and further prosthetic works. Surgically assisted orthodontic-prosthodontic combined treatment can be an effective treatment option if the anterior teeth, including dental implants, require posterior movement of the anterior segment.
